Transitions of Care
The pharmacy resident will gain fundamental knowledge and exposure to challenges associated with the transition from an inpatient to an ambulatory status as well as barriers to care in the ambulatory setting.
The pharmacy resident is responsible for evaluating medication appropriateness, assessing medication adherence, and monitoring patients for drug therapy outcomes. The resident will provide drug information to physicians, nurses, patients, and caregivers, and adequately manage communication gaps between patients and providers. In addition, the resident will conduct data assessment for outcomes of pharmacy impact on care in the transitional role. This rotation will require residents to spend 4 consecutive hours each week focusing on patient’s recently discharged from the hospital. Additionally, it will focus on the management of transitions of care for all patients with a heart failure admission during the cardiology rotation.
