Mound City Family Care

Quality healthcare couldn’t be easier or closer to home. Mound City Family Care provides multigenerational care to the families of our community. Our caregivers work to create caring relationships with patients and their families. By getting to know them and listening to their concerns and needs, we help our patients make the best healthcare decisions for them. We also offer on-premise laboratory and X-ray services.

Patient-Centered Medical Home

Mound City Family Care is a designated Level 3 Patient-Centered Medical HomeTM clinic. This is the highest level of recognition awarded by the National Committee for Quality Assurance. In a Patient-Centered Medical Home clinic, the primary care provider and patient are partners in healthcare. And this relationship is supplemented with a care team at the clinic that takes collective responsibility for the patient's care. The result is more personalized, coordinated, effective and efficient care for our patients.

At Mound City Family Care, our healthcare services are provided based on a sliding scale fee for those who qualify. Services are not denied based on inability to pay and appointments may be rescheduled to a time when a patient is able to meet their financial obligation for services. The sliding scale fee is based on your household income. A Financial Statement is required for consideration. It is important that complete and accurate information about your income is provided.
