Gardner Family Care is a designated Level 3 Patient-Centered Medical HomeTM 2011 clinic. This is the highest level of recognition awarded by the National Committee for Quality Assurance. In a Patient-Centered Medical Home clinic, the primary care provider and patient are partners in healthcare.
